Effects of Transjugular Intrahepatic Portosystemic Shunt on Renal and Pulmonary Function in Hepatic Decompensation with and without Hepatorenal and Hepatopulmonary Syndromes: A Review Ahmed H. Wu Published online: July 25, 2024 [ Html ] [ PDF ] [ Highlights ] Hepatorenal syndrome (HRS) and hepatopulmonary syndrome (HPS) are feared complications of liver cirrhosis due to their poor prognosis and limited treatment efficacy. Maladaptive vasodilatory response and systemic inflammation are important mechanisms in the pathophysiology of both syndromes. Management of HRS includes albumin and vasoconstrictors, while liver transplantation is mainly indicated for HPS management. There are data indicating improvement of renal function in patients with and without HRS. However, transjugular intrahepatic portosystemic shunt cannot be recommended solely for the treatment of HRS, and no long-term benefit has been demonstrated.
